Minnesota Heart Disease and Diabetes Community Survey

The Minnesota Department of Health wants to hear from residents across the state to learn how diabetes and heart disease affects them and their loved ones. We are sending out a short electronic survey; it is only two questions. Our goal is to make sure we hear about all the concerns and needs of diverse Minnesotans in regards to preventing and managing heart disease and diabetes. How can you help? Get the word out! Post the Community Survey link (http://j.mp/2I9zKPG) on your social media, email it to your staff, send it out to family, friends, partners and participants via email distribution lists, and respond to the survey yourself! Share the Minnesota Department of Health Facebook post (https://www.facebook.com/mnhealth/posts/2288306191215825) directly to your page. We appreciate your help.
