"Health Care Homes: A Framework for Primary Care Transformation" and ICSI Patient Activation & Engagement Three Month Learning Collaborative

ICSI and MDH have teamed up so that you can take advantage of a full day of regional learning opportunities.  Sessions for the face to face portion of the ICSI Three Month Learning Collaborative and the MDH workshops will be held in:
  • Brainerd - September 13th
  • Willmar - September 21st
  • Rochester - September 28th
  • Metro - October 1st

Participants must register separately for the ICSI and MDH offerings.
